Notice On Vaccination Dates Where Support Is Available for Foreigners

 Date and time:

1st dose September 26th (Sunday) 9 a.m. to 1 p.m., 2 p.m. to 6 p.m.
2nd dose October 17th (Sunday) 9 a.m. to 1 p.m., 2 p.m. to 6 p.m.
(You will be required to receive both the 1st and 2nd vaccination doses as 1 set.) 

Full reservations thanks.

Venue:

Asuka Hall (1st floor of the Kakamigahara City Industrial and Cultural Center) 

Applicable to those for which all of the following apply:

People with a registered residence in Kakamigahara City
People who have received a vaccination voucher from Kakamigahara City
People who are not confident in their Japanese speaking ability and who require interpreting support

Interpreters:

Major languages for the city: Filipino, Chinese, Vietnamese, Portuguese and English.

For all other languages, we will use tablet computers to provide translation. 

About reservations:

You are required to make a reservation in advance in order to be vaccinated.

We will begin taking reservations starting from 9 a.m. on September 3rd (Tuesday).

Notes:

People who are already able to speak Japanese may make their reservation in the same way as other local Japanese residents.
If you have a family member or friend who can speak Japanese, you may bring them with you when you come to get vaccinated. They will be allowed to accompany you inside of the venue.
 

Vaccinations at Tokai Central Hospital

Resumption of Vaccinations at Tokai Central Hospital

Vaccination reservations will be resumed for “individuals with recognized preexisting conditions” at the Tokai Central Hospital, and please note that we are also taking vaccination reservations for “individuals without recognized preexisting conditions” who are aged over 50.

Venue:

Tokai Central Hospital (4-6-2 Soharahigashijima-cho, Kakamigahara-shi)

Vaccination dates:

July 26th (Monday) to August 15th (Sunday)

5時30分 p.m. to 7時30分 p.m. on weekdays (conducted every day)

9 a.m. to 1 p.m. on weekends and holidays

Reservations can be made starting from 9 a.m. on July 16th (Friday).

How to make a reservation:

Reservations can be made by phone (Kakamigahara City Call Center) or through our website (Kakamigahara City COVID-19 Vaccination Reservation System).

Notes:

If you have difficulties communicating in Japanese, you may bring someone to accompany you.
We are also considering to establish a vaccination venue to be made available on a separate date at which support will be provided for foreign residents.

We accidentally sent two coupons for the COVID-19 Vaccination.
The coupons are the same.
Please use just one of them and discard the left one.

Kakamigahara City COVID-19 Vaccine Notice

Reservations are required for vaccination.

1. About the vaccine: 
Please check with the Ministry of Health, Labor and Welfare.

2. Decide on the type of inoculation:
A: Group or Mass Inoculation
Done on the reserved time, place, and date at city facilities and medical institutions.

B:Individual Inoculation
Make an appointment from the list of medical institutions.


3. Make your reservation: 
・Vaccine Reservation Schedule:
Ages 65 and up May make reservations
Those with severe illnesses May make reservations

List of Subject Persons

1. Persons who will not reached age 65 during 2021 who visit a doctor regularly or who are hospitalized for the following illnesses and conditions.
・ Chronic respiratory illness
・Chronic heart disease (including hypertension)
・ Chronic kidney disease
・ Chronic liver disease (cirrhosis, etc.)
・ Diabetes being treated with insulin or oral medicines, or diabetes with other accompanying illnesses
・ Blood diseases (excluding anemia due to iron deficienchy)
・ Diseases that reduce immunity functions (including malignant tumors under treatment)
・ Receiving treatments such as steroids that reduce immunity functions
・ Neurological diseases and neuromuscular diseases associated with immune disorders
・ States in which physical function are impaired due to neurological or neuromuscular diseases (respiratory disorders, etc.)
・ Chromosomal abnormalities
・ Severe physical and mental disabilities (when severe physical disabilities and severe intellectual disabilities overlap)
・ Sleep apnea syndrome
・ Sever mental illness (those who are hospitalized for the treatment of a mental illness,or have a Mental Patient Health Welfare Record Book, or who fall under the category of “severe and continuous” for psychiatric rehabilitation (psychiatric outpatient care)), or have an intellectual disability (with a Rehabilitation Record Book).
2. Obese people who meet the criteria (BMI of 30 or above)

 


Those with no severe existing illness May not make a reservation.
We will inform you via the city website, etc. when you may be able to make reservations for ages 64 and under with no severe existing illness
※The patient/vaccination number on your vaccination reservation form is important (you will need it).

5. Important things to remember on the day of vaccination:
Please bring:Vaccine form and pre-vaccine questionnaire (including in the envelope), health insurance card (if you have one), zairyu card, medication record book (if you have one)
Clothing:Please wear something easy to show your arm (short sleeve T-shirt, etc.)
